This report is suitable for individuals who are planning to apply for an Acupuncture license. It can also be used for admission to educational institutions.

General Required Documentation

*You must submit an Acupuncture application (Form 200) and obtain a reference# before sending ERES any documents. We are not responsible for any documents that arrive without a reference#.

*Your Form 201 and Form 202 (if you never obtained a foreign acupuncture license, then you don’t have to submit Form 202) will be generated automatically once you complete an acupuncture evaluation application online.

*All original documents must be sent from your school with its Form 201.

*If you obtained a foreign acupuncture license, then verification must be sent from the authority that issued the license in the Form 202

Requirement Details

ERES requires a precise, accurate, word by word translation for each document submitted in a language other than English.

List of acceptable translations:

  • ERES Translation
  • School-issued official Translation
  • Translation from a professional translator or translation agency

     Acupuncture Evaluation

Includes the institution(s) attended, dates of attendance, credential(s) earned, and US equivalency(ies). It also lists theoretical and practical/clinical hours for courses and a summary of curriculum hours. (It matches the CRF form)

Additional Service

Rush Service

*Acupuncture Turnaround time without rush service is 7-8 weeks.

*Payment must be made in full by Cashier’s Check, Money Order or Cash/Debit Card paying in person for rush service. The process begins the next day when complete documents and payment are received after 11am. If additional document or fee is required, the starting time will be reset.

R15 (15-Day Rush Service)

Ready for pick-up or mailing in 15 or 16 business days after receipt of complete application, documents and payment.

Fee: additional $120

R10 (10-Day Rush Service)

Ready for pick-up or mailing in 10 or 11 business days after receipt of complete application, documents and payment.

Fee: additional $180
