Supplement to the Certificate (Додаток до aтестата); Supplement to the Diploma (Додаток до дипломa) for the professional school diploma	Original ; or

For completed programs: Supplement to the Diploma (Додаток до дипломa). For incomplete programs: Academic Record (Академічна довідка) or Archival Record (Архівна довідка) issued by the Rector’s office	Original ; or
